The Surabaya Orthopedic half marathon starts and finishes at The Caribbean Grande in Pakuwon City, a large residential development in western Surabaya.

The course is described as mostly flat, making it suitable for runners targeting a personal best.

Reflecting its organizer's focus on orthopedics and fracture surgery,

the event offers separate categories by distance in addition to the general participant division,

including divisions for people recovering from knee or leg fracture surgery, medical professionals, and masters aged 45 and over.
